Process creation sequence reorganized:

- Removed instance data segment for all but NE modules.
- Check binary type in CreateProcess before loading module.
- Unix process creation moved from WinExec to CreateProcess.
- Bugfix: fill in thdb->teb.htask16 member correctly.
