Viktor Prutyanov 231f6a7d66 elf2dmp: rework PDB_STREAM_INDEXES::segments obtaining
PDB for Windows 11 kernel has slightly different structure compared to
previous versions. Since elf2dmp don't use the other fields, copy only
'segments' field from PDB_STREAM_INDEXES.

Viktor Prutyanov df7a75564e elf2dmp: use Linux mmap with MAP_NORESERVE when possible
Glib's g_mapped_file_new maps file with PROT_READ|PROT_WRITE and
MAP_PRIVATE. This leads to premature physical memory allocation of dump
file size on Linux hosts and may fail. On Linux, mapping the file with
MAP_NORESERVE limits the allocation by available memory.

Viktor Prutyanov 9b7dcd8ff4 elf2dmp: introduce merging of physical memory runs
DMP supports 42 physical memory runs at most. So, merge adjacent
physical memory ranges from QEMU ELF when possible to minimize total
number of runs.

Viktor Prutyanov 3c407ec67f elf2dmp: replace PE export name check with PDB name check
PE export name check introduced in d399d6b179 isn't reliable enough,
because a page with the export directory may be not present for some
reason. On the other hand, elf2dmp retrieves the PDB name in any case.
It can be also used to check that a PE image is the kernel image. So,
check PDB name when searching for Windows kernel image.

Akihiko Odaki 548b8edc6d elf2dmp: Don't abandon when Prcb is set to 0
Prcb may be set to 0 for some CPUs if the dump was taken before they
start. The dump may still contain valuable information for started CPUs
so don't abandon conversion in such a case.

Viktor Prutyanov d399d6b179 contrib/elf2dmp: add PE name check and Windows Server 2022 support
Since its inception elf2dmp has checked MZ signatures within an
address space above IDT[0] interrupt vector and took first PE image
found as Windows Kernel.
But in Windows Server 2022 memory dump this address space range is
full of invalid PE fragments and the tool must check that PE image
is 'ntoskrnl.exe' actually.
So, introduce additional validation by checking image name from
Export Directory against 'ntoskrnl.exe'.

Peter Maydell f015cbb546 elf2dmp: Fail cleanly if PDB file specifies zero block_size
Coverity points out that if the PDB file we're trying to read
has a header specifying a block_size of zero then we will
end up trying to divide by zero in pdb_ds_read_file().
Check for this and fail cleanly instead.

Peter Maydell ddb45afbfb contrib/elf2dmp: Build download.o with CURL_CFLAGS
contrib/elf2dmp has a source file which uses curl/curl.h;
although we link the final executable with CURL_LIBS, we
forgot to build this source file with CURL_CFLAGS, so if
the curl header is in a place that's not already on the
system include path then it will fail to build.

Add a line specifying the cflags needed for download.o;
while we are here, bring the specification of the libs
into line with this, since using a per-object variable
setting is preferred over adding them to the final
executable link line.

Markus Armbruster 37677d7db3 Clean up a few header guard symbols
Commit 58ea30f514 "Clean up header guards that don't match their file
name" messed up contrib/elf2dmp/qemu_elf.h and
tests/migration/migration-test.h.

It missed target/cris/opcode-cris.h and
tests/uefi-test-tools/UefiTestToolsPkg/Include/Guid/BiosTablesTest.h
due to the scripts/clean-header-guards.pl bug fixed in the previous
commit.

Commit a8b991b52d "Clean up ill-advised or unusual header guards"
missed include/hw/xen/io/ring.h for the same reason.

Commit 3979fca4b6 "disas: Rename include/disas/bfd.h back to
include/disas/dis-asm.h" neglected to update the guard symbol for the
rename.

Commit a331c6d774 "semihosting: implement a semihosting console"
created include/hw/semihosting/console.h with an ill-advised guard
symbol.

Clean them up.

Header guard symbols should match their file name to make guard
collisions less likely.

software.  The remaining reverts are the same as in commit
b7d89466dd.

Viktor Prutyanov 06164cc402 contrib/elf2dmp: add kernel start address checking
Before this patch, if elf2dmp failed to find NT kernel PE magic in
allowed virtual address range, then it assumes NULL as NT kernel
address and cause segfault.

This patch fix the problem described above by checking NT kernel address
before futher processing.

Paolo Bonzini 7d37435bd5 avoid TABs in files that only contain a few
Most files that have TABs only contain a handful of them.  Change
them to spaces so that we don't confuse people.

disas, standard-headers, linux-headers and libdecnumber are imported
from other projects and probably should be exempted from the check.

Viktor Prutyanov 3fa2d384c2 contrib: add elf2dmp tool
elf2dmp is a converter from ELF dump (produced by 'dump-guest-memory') to
Windows MEMORY.DMP format (also know as 'Complete Memory Dump') which can be
opened in WinDbg.

This tool can help if VMCoreInfo device/driver is absent in Windows VM and
'dump-guest-memory -w' is not available but dump can be created in ELF format.

The tool works as follows:
1. Determine the system paging root looking at GS_BASE or KERNEL_GS_BASE
to locate the PRCB structure and finds the kernel CR3 nearby if QEMU CPU
state CR3 is not suitable.
2. Find an address within the kernel image by dereferencing the first
IDT entry and scans virtual memory upwards until the start of the
kernel.
3. Download a PDB matching the kernel from the Microsoft symbol store,
and figure out the layout of certain relevant structures necessary for
the dump.
4. Populate the corresponding structures in the memory image and create
the appropriate dump header.
