monitor: Create MonitorHMP with readline state

The ReadLineState in Monitor is only used for HMP monitors. Create
MonitorHMP and move it there.

Can't use container_of() in hmp_change().  Cast instead, and mark
FIXME.  Will be cleaned up shortly.
