[SCSI] fas216: Use scsi_eh API for REQUEST_SENSE invocation

Use new scsi_eh_prep/restor_cmnd() for synchronous REQUEST_SENSE
invocation.  This also converts the driver to the new accessor based
scatterlist implementation.
